It outlines the project scope, objectives, team members, and timeline, providing a clear roadmap for project execution. Our Project Charter Template includes the following sections:

  1. Project Details:
    • Project Name, ID Number
    • Black/Green Belt Name, Sponsor Name
    • Start Date, Projected Annual Savings, Type of Savings
    • Team Leader, Telephone Number, Target Completion Date
  2. Opportunity Statement:
    • A brief description of the problem or opportunity the project addresses.
  3. Project Objectives:
    • Specific, measurable, attainable, relevant, and time-bound goals.
  4. Business Case:
    • The rationale and urgency for the project.
  5. Approach:
    • The methodology to be used (e.g., DMAIC, Lean, Just-Do).
  6. Opportunity Definition:
    • Describe what provides the opportunity to produce (or NOT produce) a defect. For example, a customer transaction that may or may not have a mistake, a patient treatment that may or may not have a complication, an item produced that may or may not be defective.
  7. Defect or Error Definition:
    • What is an unacceptable value for the metric? How would you describe it when things go wrong? For example, a customer transaction done incorrectly, a line of code with an error, a patient treatment that results in a complication, a defective item produced.
  8. Metrics for DPMO Calculations:
    • Metrics before and after the project.
  9. Scope:
    • Process Scope: Work process impacted, start and end points.
    • Project Scope: In-scope and out-of-scope work.
  10. Potential Adverse Impacts:
    • Possible unintended consequences to monitor.
  11. Potential Barriers:
    • Anticipated obstacles and challenges.
  12. Milestones:
    • Key project milestones and dates.
  13. Core Team Members:
    • Team members and their roles (DACI – Decision, Approve, Consult, Inform).
